Check balls omitted or not positioned correctly

Inspect the valve body.

Internal damage to torque converter

Replace the torque converter.

Direct clutch piston or seal leaking or damaged

Inspect the direct clutch piston and seal.

Incorrect fluid pressure

Perform the line pressure diagnosis.
