SEAT BELT EXTENSIONS REVISED ODERING PROCEDURE

VEHICLES AFFECTED: All Passenger Vehicles Excluding Corvette

In cases where an existing seat belt cannot be buckled up, due to insufficient length, a seat belt extension is available at no charge to the owner. Orders for seat belt extensions should not be submitted if it is determined that the intent is simply to provide a vehicle occupant with more slack for the sake of comfort.

Effective immediately, a new procedure and a three part order form must be used to handle any requests.

The seat belt extension will be custom made for the individual and vehicle involved and personalized with an attached label.

The label will contain the individual's name and the identification number of the vehicle.

CAUTION:

Use of the extension by anyone but the person for whom it was designed or in a vehicle other than the one specified may increase the chance of and/or severity of an injury in the event of an accident. To ensure that the applicant is aware of the correct usage of the extension, the request form must be signed by the APPLICANT and requests limited to one per order form. All extensions will be supplied in black. This will expedite the orders and will also identify the extension as nonstandard equipment to deter misuse by other vehicle occupants.

Enclosed is a sample of the new order form which details the procedure to be used.

All requests should be handled as follows:

1. Arrange to have the order form completed ensuring that all pertinent information is obtained, and that the form is signed by the applicant.

2. Contact your Zone Service Department for approval of the request.

3. Upon completion of the form, THE YELLOW COPY MUST BE RETAINED ON FILE AT THE DEALERSHIP and the remainder forwarded to the Zone office.

4. The custom made extension will be returned to the Dealership for delivery to the customer.
